The Clinical Social Worker/Mental Health Counselor provides and coordinates psychosocial and behavioral services for her/his group of patients. She/he assesses, plans, implements and monitors a plan of care for her/his group of patients. This is done in collaboration with other members of the pediatric psychiatry collaborative Hub team and pediatric primary care practices. Works with the care team in a designated setting to identify, and, when possible, advocates for the patient. Works to provide direct services when necessary, resources and counseling that a patient/client needs to create a safe, effective and efficient care plan. Provides his/her expertise in a crisis intervention situation and assesses for potential abuse/neglect of vulnerable patients. Provider will assess the needs of and provide training and technical assistance to pediatric primary care practices and behavioral health organizations. Provider will interface with families of children referred over for consultation, obtain additional rating scales and historical information, provide case management support for referrals for treatment resources, and provide other case management support for resources to families. This position will also provide brief family counseling and behavioral counseling; maintains electronic files on consultation activity.
Job Opening Qualifications
Minimum Education and Experience Requirements:
Master's degree in social work (MSW) or master's degree in mental health counseling (MHC).
Professional certification in specialty area preferred.
Recent clinical experience applicable to the designated population and/or served.
Experience in a health care setting preferred.
Motor Vehicle Operator Designation:
