To bridge the gap in care that often takes place when patients transition from one healthcare setting to another, Dr. Mann formed Synergy Medical Group. The idea is for every piece of a person’s healthcare journey to flow seamlessly and smoothly, with synergy among the multidisciplinary groups and settings. When your healthcare team is able to follow along with you across the different levels of care, this gap during the transition period is nonexistent and continuity of care is promoted. Dr. Mann started this group as the sole physician and has built it from the ground up, to a state-wide group servicing various types of facilities and patients in the outpatient setting.
